Confidence in risky value-based choice
Risk engenders a phenomenologically distinct experience from certainty, often driving people to behave in ostensibly irrational ways, and with potential consequences for our subjective sense of confidence in having made the best choice. While previous work on decision confidence has largely focused on ambiguous perceptual decisions or value-based choices under certainty, it is unclear how subjective confidence reports are formed during risky value-based choice (i.e. those with uncertain outcomes). Accordingly, we sought to examine the effect of risky (versus certain) choice upon confidence ratings in a calibrated economic choice task and explore the well-documented interrelationships between confidence and subjective value (SV) as well as choice response time (RT) in the context of value-based choice. By jointly analyzing choices (risky versus certain), SV of the chosen option, confidence, and RT, we found a systematic effect of risk on subjective confidence: subjective confidence reports were significantly higher when selecting a certain prospect compared with a risky one. Interestingly, risk attenuated the strength of the relationships between confidence and both RTs and difference in subjective value (ΔSV), as well as the relationship between RT and ΔSV. Taken together, these results corroborate how choice, RT, confidence and SV relate in value-based choice under risk, informing both theories of confidence and risk preferences

Interventions to prevent urinary catheter-associated infections in children and neonates: a systematic review.
INTRODUCTION: Few data are available to inform strategies for the prevention of catheter-associated urinary tract infection (CAUTI) in children and neonates. Many recommendations are derived from studies in adults and cannot be applied to the paediatric population. OBJECTIVE: This study was aimed to identify all studies that measured the efficacy of an intervention for the prevention of CAUTI in children and neonates. METHODS: A systematic review using the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) was conducted. Eligible studies published between January 1st, 1995 and December 31st, 2017, were identified in PubMed, the Cochrane Database of Systematic Reviews, LILACS, SciELO and DOAJ if applying an intervention with the aim of CAUTI prevention in inpatient children, infants or neonates. The following study designs were included: controlled and non-controlled before-and-after studies, (controlled) interrupted time series analyses and randomized controlled trials. Quantitative or qualitative studies on interventions in both adults and children were eligible if data on children could be extracted. Reviews, case series, letters, notes, conference abstracts and opinion articles were excluded. RESULTS: Of 99 articles identified, six were included in the final analysis, after consensus from three independent investigators. Four studies used a multimodal strategy (using at least four or more different components at the same time) as follows: aseptic rules during catheter insertion and removal; cleaning the urethral meatus with sterile water; use of a new silicone catheter per insertion with a closed sterile drainage system by a sterile technique; daily evaluation of catheter requirement; placement of indwelling urinary catheters only for approved indications; reducing of urinary catheter days and positioning of the patient and collection device to assist in urine drainage. One study tested periurethral cleaning intervention to reduce CAUTI. One study described the association of the presence of a physician safety champion with urinary catheter device utilization ratios. Catheter-associated UTI reduction rates were reported in four studies; three achieved statistically significant decreases in CAUTI rates. Positive results were achieved only when a multimodal strategy was used with at least four or more components. This strategy could be adopted for paediatric healthcare institutions to reduce CAUTI rates in children and neonates. CONCLUSION: Evidence exists to support the use of a multimodal strategy for CAUTI reduction in hospitalized children and neonates
Drug Release from Microspheres and Nanospheres of Poly(lactide-co-glycolide) without Sphere Separation from the Release Medium

Studies of chloro(5,10,15,20-tetraphenylporphyrinato)indium(III) (InTPP) release from microspheres and nanospheres of poly(lactide-co-glycolide) (PLGA) were performed using this new technique. The release of InTPP was biphasic, with an initial fast release followed by a second slower release. Mathematical models applied to the release profiles showed that the release of InTPP from the nanospheres was controlled by diffusion, which is to be expected for a substance homogeneously dispersed within the spheres. However, due to the large size distribution of the microspheres loaded with InTPP, the release profiles were irregular, hampering an adequate fit to mathematical models. Dapagliflozin Impact on the Exercise Capacity of Non-Diabetic Heart Failure with Reduced Ejection Fraction Patients
Background: Dapagliflozin has been shown to reduce morbidity and mortality in Heart Failure with reduced Ejection Fraction (HFrEF), but its impact on exercise capacity of non-diabetic HF outpatients is unknown.
Methods: Adult non-diabetic HF patients with a left ventricular ejection fraction (LVEF) <50% were randomized 1:1 to receive dapagliflozin 10 mg or to continue with HF medication. Patients underwent an initial evaluation which was repeated after 6 months. The variation of several clinical parameters was compared, with the primary endpoint being the 6 month peak oxygen uptake (pVO2) variation.
Results: A total of 40 patients were included (mean age 61 ± 13 years, 82.5% male, mean LVEF 34 ± 5%), half being randomized to dapagliflozin, with no significant baseline differences between groups. The reported drug compliance was 100%, with no major safety events. No statistically significant difference in HF events was found (p = 0.609). There was a 24% reduction in the number of patients in New York Heart Association (NYHA) class III in the treatment group as opposed to a 15.8% increase in the control group (p = 0.004). Patients under dapagliflozin had a greater improvement in pVO2 (3.1 vs. 0.1 mL/kg/min, p = 0.030) and a greater reduction in NT-proBNP levels (-217.6 vs. 650.3 pg/mL, p = 0.007).
Conclusion: Dapagliflozin was associated with a significant improvement in cardiopulmonary fitness at 6 months follow-up in non-diabetic HFrEF patients.info:eu-repo/semantics/publishedVersio

Adjustment of the Arabidopsis circadian oscillator by sugar signalling dictates the regulation of starch metabolism.
Arabidopsis plants store part of the carbon fixed by photosynthesis as starch to sustain growth at night. Two competing hypotheses have been proposed to explain this diel starch turnover based on either the measurement of starch abundance with respect to circadian time, or the sensing of sugars to feedback to the circadian oscillator to dynamically adjust the timing of starch turnover. We report a phase oscillator model that permitted derivation of the ideal responses of the circadian regulation of starch breakdown to maintain sucrose homeostasis. Testing the model predictions using a sugar-unresponsive mutant of Arabidopsis demonstrated that the dynamics of starch turnover arise from the circadian clock measuring and responding to the rate of change of cellular sucrose. Our theory and experiments suggest that starch turnover is controlled by the circadian clock acting as a dynamic homeostat responding to sucrose signals to maintain carbon homeostasis
Antifouling Marine Coatings with a Potentially Safer and Sustainable Synthetic Polyphenolic Derivative
The development of harmless substances to replace biocide-based coatings used to prevent or manage marine biofouling and its unwanted consequences is urgent. The formation of biofilms on submerged marine surfaces is one of the first steps in the marine biofouling process, which facilitates the further settlement of macrofoulers. Anti-biofilm properties of a synthetic polyphenolic compound, with previously described anti-settlement activity against macrofoulers, were explored in this work. In solution this new compound was able to prevent biofilm formation and reduce a pre-formed biofilm produced by the marine bacterium, Pseudoalteromonas tunicata. Then, this compound was applied to a marine coating and the formation of P. tunicata biofilms was assessed under hydrodynamic conditions to mimic the marine environment. For this purpose, polyurethane (PU)-based coating formulations containing 1 and 2 wt.% of the compound were prepared based on a prior developed methodology. The most effective formulation in reducing the biofilm cell number, biovolume, and thickness was the PU-based coating containing an aziridine-based crosslinker and 2 wt.% of the compound. To assess the marine ecotoxicity impact of this compound, its potential to disrupt endocrine processes was evaluated through the modulation of two nuclear receptors (NRs), peroxisome proliferator-activated receptor gamma (PPAR gamma), and pregnane X receptor (PXR). Transcriptional activation of the selected NRs upon exposure to the polyphenolic compound (10 mu M) was not observed, thus highlighting the eco-friendliness towards the addressed NRs of this new dual-acting anti-macro- and anti-microfouling agent towards the addressed NRs

This study describes the ultrasonographic techniques currently used in the evaluation of the canine pancreas. Ultrasonography was the first method to enable direct visualization of the pancreas in humans and it has been subsequently applied to animals. Currently, it is the method of choice for pancreatic evaluation and is essential as a diagnostic tool in the detection of abnormalities, especially tumors. Innovative equipment technology has led to the emergence of techniques complementary to B-mode ultrasound; such as Doppler, elastography, and contrast-enhanced ultrasonography, which have enabled more accurate diagnosis. Doppler provides information on vascular architecture and the hemodynamic aspect of blood vessels in multiple organs. ARFI elastography provides detailed images of the alterations detected by conventional examination (qualitative method) and assists in differentiating between benign and malignant processes (quantitative method). Microbubble contrast agents determine parameters related to homogeneous and heterogeneous filling of organs with microbubbles, mainly nodular areas, thus defining high and low intensity patterns.Este estudio describe las técnicas ecográficas frecuentemente utilizadas para evaluar el pancreas del perro. La ecografía fue el primer método que permitió la visualización directa del páncreas en seres humanos y que luego se aplicó en animales. Es actualmente el método de elección para la evaluación del páncreas y es esencial como herramienta diagnóstica en la detección de anomalías, especialmente tumores. La tecnología innovadora de los equipos, llevó a la aparición de técnicas complementarias al modo B, tales como Doppler, elastografía, ecografía de contraste, que han permitido realizar diagnósticos más precisos. El Doppler proporciona información sobre la arquitectura vascular y aspectos hemodinámicos de los vasos sanguíneos en múltiples órganos. La elastografía ARFI ofrece imágenes detalladas de las alteraciones detectadas en exámenes convencionales (método cualitativo) y ayuda a diferenciar entre procesos benignos y malignos (método cuantitativo). Los agentes de contraste con microburbujas permiten determinar parámetros relacionados con el llenado homogéneo o heterogéneo de los órganos, principalmente áreas nodulares, definiendo, por tanto, patrones de alta o baja intensidad
